Cheek fillers: 48-Hour Aftercare Do’s and Don’ts

By AKKI Aesthetics | September 21, 2026

Service spotlight: Cheek Filler (ML) (about 10 minutes, $599) at my Williston Park location near the Willis Avenue commercial corridor.

Cheek filler (ML) aftercare guide with 48-hour do's and don'ts for cheek fillers

The most important rule after cheek fillers is simple: keep pressure and “messing with it” off your cheeks for the first 48 hours. I hear the same thing from a lot of first-timers, they expect soreness or a rough recovery, then they’re surprised it feels easier than they imagined. Still, the way you treat your face in those first two days makes a real difference in swelling, bruising, and how clean your result settles.

0 to 48 hours after Cheek Filler (ML): the rules that protect your result

Cheek Filler (ML) is quick, plan for about 10 minutes. After that, your job is to let the filler settle without extra heat, pressure, or inflammation. If you’re heading back to Mineola, Garden City, East Williston, Albertson, Roslyn, or Westbury, these steps are easy to follow even on a school-run schedule or after a long workday commute.

Do this

  • Cold packs on and off for 10 to 15 minutes at a time during the first day if you’re puffy or tender. Don’t press hard. Think gentle, not aggressive icing.
  • Sleep with your head elevated for the first couple nights. Extra pillows help keep swelling down.
  • Keep your face contact light. Gentle cleansing is fine. Skip heavy rubbing, face tools, and resting your cheek in your hand.
  • Use SPF when outdoors. Sun protection matters even when you’re just running errands near the Willis Avenue commercial corridor.

Skip this for 24 to 48 hours

  • Strenuous exercise. Elevated heart rate tends to worsen swelling and bruising right away.
  • Alcohol and blood-thinning supplements (unless your prescribing doctor told you to take them). This is one of the easiest ways to reduce bruising risk.
  • Heat like saunas, steam rooms, hot yoga, and very hot showers right on the face.
  • Massage or “evening it out” unless I specifically tell you to. Most of the time, hands-off is the smartest move.

If you’re planning your appointment around the LIRR or Northern State Parkway, I often recommend late-afternoon visits so you can go home, keep your head elevated, and keep the evening simple.

Days 3 to 7: how cheek filler usually settles (and what to avoid)

This is the week where people can overthink every mirror check. A little swelling, mild bruising, and some firmness can be totally normal. One side can look slightly different than the other while things calm down. That doesn’t automatically mean something is wrong.

Keep doing: gentle cleansing, light skincare, daily SPF, and sleeping so your cheek isn’t smashed into the pillow. If you can, avoid long phone calls with your cheek pressed to the phone and avoid tight helmet straps that sit on the midface.

Avoid for at least 2 weeks: facials, laser, chemical peels, and microneedling. Those are great services in the right timing, but not while you’re still settling a filler result. Give your cheeks the quiet time they need.

And keep your plans realistic. If you’re headed to a weekend event at Eisenhower Park or you’ve got photos coming up, schedule with a little buffer. You might look great right away, but bruising has its own schedule.

Simple products and habits I like for cheek fillers (no fancy routine needed)

After cheek fillers, your skincare routine should feel boring for a bit. That’s a good thing. Stick with gentle cleanser, a basic moisturizer, and sunscreen. If you’re dry, hydration helps your skin look smoother while the filler settles.

Cold compresses and arnica

Cold packs (gentle, short intervals) are usually the easiest win in the first day. If you bruise easily, arnica is a common option people use to help bruising fade. If you’re on medications or you’re pregnant, check what’s appropriate for you first.

SPF is non-negotiable

Sun can make bruising look darker and last longer. Use SPF when you’re outside, even if you’re just making quick stops around Williston Park.

Skip “active” products briefly

For a few days, I generally prefer you pause irritating actives if your skin tends to react. If something stings or makes you red, give it a break and keep it gentle.

If you’re also planning other treatments like botox, jawline filler, chin dermal filler, or even microneedling, I’ll help you space them out so your results look intentional and your skin has time to calm down.

When I want you to reach out to me (normal vs not-normal)

I take follow-up seriously. People come to me because they want a natural result and clear communication, and that doesn’t stop when you walk out the door. If something feels off, I’d rather you ask than sit at home spiraling.

Usually normal

  • Mild swelling or bruising that slowly improves
  • Some tenderness when you press on the area
  • Temporary firmness that softens as it settles

Reach out right away

  • Increasing pain (not just soreness)
  • Growing redness, warmth, or drainage
  • Skin that looks pale or blanched in a spot
  • Hard lumps that are getting worse, not better

“I was nervous at first but he talked me through the whole process and made me feel at ease.”

one of our first-time clients

I also like a check-in around 1 to 2 weeks so we can look at how everything settled. A lot of people appreciate that kind of follow-through. If you’ve been searching “fillers near me” or “med spa near me” and you want a place that doesn’t rush you, I’d love to see you at AKKI Aesthetics in Williston Park.

Quick reading if you’re planning more than one injectable

Cheek filler often pairs nicely with other face fillers, but timing matters. If you’re spacing out botox and filler, or you’re thinking about fillers for smile lines, I recommend you keep a simple calendar for the next two weeks so you don’t accidentally schedule a facial or laser too soon.
